As a Community Support Specialist, you will be providing ongoing, long-term support to individuals with disabilities after they have completed job coaching and transitioned to independent work. The CSS ensures job retention by conducting regular follow-ups, collaborating with employers, and addressing workplace challenges before they become barriers to employment.
Essential Duties and Responsibilities:
Employment Support & Skill Development
- Assist clients in adjusting to the workplace and developing independence.
- Provide ongoing support to promote job retention.
- Teach self-advocacy, workplace communication, and problem-solving skills.
- Reinforce positive workplace behaviors and build confidence for job success.
- Identify and address challenges that may impact job stability.
Follow-Ups, Employer Collaboration & Crisis Management
- Conduct check-in twice a month to assess job performance and satisfaction
- Collaborate with clients and employers to resolve workplace concerns.
- Ensure clients fully understand job expectations, responsibilities, and company policies.
- Address workplace challenges early to prevent escalation and find solutions beneficial to both the client and the employer.
Person-Centered Planning & Case Management
- Annually update the Person-Centered Plan to reflect employment goals, challenges, and support strategies.
- Monitor job retention and performance, adjusting support as needed.
- Maintain accurate records of client progress and service provided.
- Conduct and document Long-Term Follow-Up Progress Reviews.
- Submit the required reports and service hours to Trillium and other funding agencies.
Other
- May assist Employment Specialist with Intakes and training as needed.
Requirements:
- Bachelor’s degree required in a human services-related field (e.g., social work, psychology, counseling, etc.) and:
- At least two years of full-time, post-bachelor’s supervised experience in mental health/developmental disabilities/substance abuse (MH/DD/SAS) populations or supervised experience in substance abuse counseling. ?
- If a bachelor’s degree is in a non-human services field, four years or supervised experience in MH/DD/SAS populations or supervised experience in substance abuse counseling is required.
- Master’s degree in a human services-related field preferred, with at least one year of full-time, post graduate supervised experience in MH/DD/SAS populations or substance abuse counseling.
Other Requirements
- Valid driver’s license and safe driving record
- Strong communication and documentation skills.
- Case management experience preferred.
- Flexibility to work occasional evenings, weekends, and holidays.
- Must meet the definition of a Qualified Professional under the NC MH/DD/SAS.
